refer to choices.

How can professionals refer young people into Choices services? Please note that for Choices to work with a young person we must have the consent of the young person as our service is voluntary. A referral can be made online via this website on our professionals page, using the “refer a young person” and “refer a parent” tabs. This will download our referral forms that can then be sent to choiceswest@barod.cymru . Alternatively, you can contact our SPOC number as shown below and a member of the team will be able to complete a referral over the form with you. Please indicate on the referral if you would like feedback.
